If you have already filled this out, or worked with a board member to fill it out, please ignore.
If you do not fill this out, we cannot move you forward with the rest of the complainants. We have really struggled to figure out exactly where all the members are at in the application process. So far, filling this document out has been the most concise way for us to get the information we need from all the complainants
If you fill this out, and for those who already have, we cannot guarantee any level of legal success. Only that is helps coordinate current and future activities.
